Jr Permission to Tryout Forms
Those players wishing to tryout for a Junior team will require a 2016-17 OMHA Junior Player Tryout Permission Form for each team whose tryouts they plan to attend. These forms will be available from Tracy Harness at the Eagles office during
office hours starting Saturday March 26, 2016.
YOU MUST OBTAIN A FORM FOR EVERY TEAM WHOSE TRYOUTS YOU WILL BE ATTENDING
