Major Midget Tryout Permission Form

John Tait will be signing Major Midget Tryout Permission Forms on August 23, 2014 between 10:00am and 11:00am at the Burlington Eagles office.  Major Midget players wishing to Tryout at another Centre will require this form.  Please stop by the office during that time to pick one up.  If you have any questions please email [email protected]
