2016-17 Major Midget OMHA Permission To Skate
John Tait will be signing Major Midget Tryout Permission Forms on Saturday August 20, 2016 between 9:00 am and 11:00 am at the Burlington Eagles office. Major Midget players wishing to attend tryouts at another Centre after August 23 will require this signed form. Please stop by the office during that time to pick one up. [email protected]All players who did not play in Burlington last season and reside outside of Burlington will require a Major Midget AAA Player Tryout Permission Form (Permission to Skate) from their home center to attend our Tryouts. As per direction from the OHF, all associations will have the forms and must provide them upon request. Please note this is NOT the same form as the Permission to Skate form required to try out for Junior teams.
